FORMS

Reimbursement/Check Request Form - Use this form if you have made a purchase for an AVHSC expense and you need to be reimbursed. (ex: Room parents funds, class party, drama club purchase, purchases made for school events, etc.)

Tax Donation Form/Tax Donation Receipt Request - Use this form if you have made a donation to the Alta Vista Home and School Club and you need a receipt for tax Purposes

Deposit Request Form - Use this form if you have cash from a school event that you need to give the AVHSC treasurer

Cash Box Request Form - Use this form if you need to request cash needed for a school event or other activity. (This is typically for event chairs or committee members)

Grant Request Form

The AVHSC Grant Program allows its members (teachers, staff, and parents) to request AVHSC funds to fund supplemental programs for students and provides the opportunity to fund new enrichment programs that benefit our school community.

How to submit a Grant Request:

1. Please review the 2024-2025 Grant Program Guidelines document before you submit your request.

2. Submit your grant request online via Google Forms.

Grant applications will be reviewed every month at the AVHSC meeting and you will be notified if your grant was approved within 3 business days after the HSC meeting. If you have any questions please contact the Ryan Greco at rpgreco@gmail.com.
